Your First Visit
First Visit

Your first visit to our office will be for a consultation, routine treatment or emergency treatment.  This would have been determined and scheduled appropriately at the time of your initial phone call to us. 

Please assist us by providing the following information when you arrive:

  • Any information your dentist might have given you to bring to us, such as a referral slip and radiographs.  
  • A list of medications you are presently taking.
  • If you have dental insurance, bring the necessary information and insurance cards. This will save time and allow for prompt filing of claims.

If you have not been to our office before, or if it has been a number of years since your last visit, please arrive about 15 minutes earlier than your scheduled time to allow for paperwork.  We try to be very prompt and ask our patients to do the same.  If we realize we are running late, if you have provided us with your phone numbers (sometimes a cell phone number is the best), we will try to reach you, if possible.

IMPORTANT: A parent or guardian must accompany all patients under 18 to each visit.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), if you have any allergies (i.e. medications, antibiotics, latex, etc.), if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e antibiotics, for premedication) 

 

If you have a medical condition which requires an antibiotic be taken before dental treatment, please remember to premedicate as directed.  If you are not sure, please call us at Manchester Office Phone Number603-624-9786.
